Possible to create a bookmark tag with no name

Originally reported on Google Code with ID 3634
A user somehow created a bookmark with one tag which had no name. Because the tag was
nameless, links to it were causing routing errors, making the unsorted tags page inaccessible.
We need to figure out how it was possible to get that to happen and how to prevent
if from happening in the future.

(The bookmark was for an internal work and had no notes.)

It just happened again, but this time, the unsorted page did not break. There is a blank
tag on page 10 right now, between "derr" and "sydrian". Clicking the "Bookmarks" link
gets this: http://archiveofourown.org/tags//bookmarks

This returns the "Bookmarks" freeform tag, rather than the blank tag.
